4 Full-time 5 hours ago Full Job Description Under the general direction of the Manager, the Lead RN Case Manager - Point of Entry leads the Point of Entry (POE) Case Management team in the Emergency Department.
Responsible for ensuring that POE Case Managers screen all patients for medical necessity of admission by using industry-approved Level of Care Criteria.
Assists the Department Manager with hiring, scheduling, monitoring, orienting, and training POE Case Managers and support staff.
Also functions as a Point of Entry Case Manager, as needed, and assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.
Education/Training:
Graduate of NLN Accredited School of Nursing, BSN required.
Ability to read, write, and communicate in English.
Experience:
Minimum of five years of progressive experience working in a hospital setting and at least 2 years of lead or supervisory experience in acute care, preferably in case management or utilization management.
Must have experience forming positive, collaborative relationships with hospital staff, providers, patients, and families.
Must demonstrate critical thinking skills to find solutions to problems, work independently, manage stress, prioritize work, and maintain calm demeanor in stressful environment.
Licensure, Registration or Certification:
Current New York State Licensure as a Registered Professional Nurse in the State of New York.
Certification in Case Management required within 18 months of hire.
NYS PRI and Screen certification required within 3 months of hire.
