A health services network in New York is currently seeking an experienced, licensed Registered Nurse (RN) to join their team in Orange County as their new Lead Case Manager.
In this role, the Lead Case Manager (RN) will be responsible for leading the Point of Entry (POE) Case Management team in the Emergency Department.

Responsibilities:
The Lead Case Manager (RN) will be responsible for:
Ensuring that POE Case Managers screen all patients for medical necessity of admission by using industry-approved Level of Care Criteria Assisting the the Department Manager with hiring, scheduling, monitoring, orienting, and training POE Case Managers and support staff Functioning as a Point of Entry Case Manager and assuming accountability for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs Performing other duties, as needed
Qualifications:
5
years of experience working in a Hospital setting; 2
years of leadership experience in an Acute Care role/setting Bachelor's Degree in Nursing NYS Registered Nurse (RN) license Certification in Case Management required 18 months after eligibility NYS PRI and screen certification required within 3 months of hire Desired Skills:
Leadership and/or Supervisory experience in a Case Management or Utilization Management Recommended Skills Case Management Certified Nurse Practitioner Critical Care Leadership Nursing Scheduling Estimated Salary: $20 to $28 per hour based on qualifications.
